I, the parent/guardian of the above-named child, acknowledge and understand that participation in Grassroots FC programs involves physical activity and inherent risks, including but not limited to collisions, falls, muscle strains, joint injuries, illness, and other physical injuries or trauma. I understand that injuries may occur even when proper supervision, safety measures, and reasonable precautions are in place.

I hereby give consent for my child to participate in all Grassroots FC programs and confirm that my child is physically fit to participate. I have disclosed to Grassroots FC coaching staff all known medical conditions, allergies, injuries, or other relevant health information concerning my child.

In consideration of my child's participation, I voluntarily assume all risks associated with such participation and release and hold harmless Grassroots FC, its coaches, staff, volunteers, and facility operators from any claims, damages, losses, or liabilities arising from or related to my child's participation, including injury, illness, or property damage, except in cases of gross negligence or intentional misconduct.

I authorize Grassroots FC coaching staff to seek emergency medical treatment for my child if they are injured or become ill, to arrange transportation to a medical facility when reasonably necessary, and to provide relevant medical information to emergency personnel. I understand that I am responsible for all medical expenses and costs incurred as a result of such treatment or transportation.

I agree that my child will follow all instructions given by Grassroots FC coaching staff, behave appropriately and safely, and refrain from dangerous or disruptive conduct. I also accept responsibility for ensuring my child is picked up promptly at the end of each session.

I understand that photographs or videos of my child may be taken during Grassroots FC programs and may be used for Grassroots FC's social media, promotional, and marketing purposes.
